Please note: participation to the Postgraduate course is limited. Priority will be given to application by participants registering also to the 1st European Endoscopic Surgery Week.

Complete the registration form or register on line for the Congress and the Postgraduate Course. Make sure that you follow the instructions for payment of all fees involved. The registration form is for one delegate only and his/her accompanying person(s). Completed registration should be sent to

Delegates are encouraged to register on line.

Payment.

Payment should be made in Euro (€) in advance by:

• Banker’s Draft, which should be sent together with the registration form.
• Holders of American Express, Visa or Mastercard may use their credit card. Card number, expiration date, CVC II code, name of card holder with address and signature must be filled in.
• Bank transfer: (after receipt of the undersigned registration form delegates will receive a confirmation with registration number and invoice for the appropriate fees with banking details).

Remittances should be free of bank charges to the receiver. The registration form must indicate the means of payment used. All payment must be clearly marked with the delegate’s name and made payable to Endo-Surgery Week (Management) Ltd.

Upon receipt of the completed registration form or on line registration and remittance of the appropriate fees, delegates will be sent a final confirmation letter, which should be presented at the registration desk in Glasgow.

Cancellation and Refunds.

Notification of cancellation must be sent in writing to the EAES Office. Cancellation will be accepted until May 5, 2003 with a refund of all registration fees less €35 administration fee. No refunds can be made for cancellations after May 5, 2003 or for participants who do not attend. All refunds will be handled after the congress
