Registration

Early Registration up to 20^th January 2004;
Late Registration after 20^th January 2004

Registrations should be made on the official Form and sent to the Conference Secretariat t the address on the left, with full payment. (Photocopies of the Form will be accepted).

Registration and payment can be made using the form that can be download via the link to the left. An online facility will be available shortly.

Tickets for the Gala Dinner must be pre-ordered on the Form.

The Registration Fee includes: Participation in the Scientific Sessions, Poster Viewing and Exhibition, Conference Programme and Abstract Book, lunch, teas/coffee and VAT where applicable.

Late Registration (after 20^th January 2004) £180.00

Payments

1. Payment must be made in full, in pounds sterling either by:
   1. cheque drawn on a UK bank,

      IT IS ABSOLUTELY ESSENTIAL THAT FULL DETAILS OF ANY BANK TRANSFER ARE PROVIDED WITH THE REGISTRATION FORM. IT IS ONLY POSSIBLE TO ALLOCATE RECEIVED FUNDS CORRECTLY WITH THIS DOCUMENTATION SO IT IS IN THE INTERESTS OF ALL PAYEES USING THIS METHOD. FUNDS SHOULD BE TRANSFERRED AT LEAST 30 DAYS PRIOR TO THE CONFERENCE TO ENSURE CONFIRMATION OF REGISTRATION.
2. Credit Cards: by using one of the following cards: Visa, Mastercard or Switch.
   PLEASE NOTE, THAT IF YOU PAY BY CARD, A 4% ADMINISTRATION CHARGE IS PAYABLE ON ALL CARD TRANSACTIONS TO COVER CARD COMPANY CHARGES.

Confirmation

Only registrations received with full payment will be registered for the Conference.

Cancellation

All cancellations should be made IN WRITING to the Conference Secretariat. Cancellations received before 30th January 2004: full refund minus 25% administration charge: Cancellation after 31st January 2004 no refund. All approved refunds will be processed and issued 60 days after the Conference.
